Now the risk starts at this point, as the Captain and Ship in charge should have consulted on weather predictions before they set out. The local people were not informed in advance about the weather either. Shipping wreck issues during storms was not a new thing, and hence the failure to check for weather status before departure can be considered typical root cause failure. What happened in the aftermath of the gale were subsequent failures. Now although data is not available on why the Ship attempted to reach NSW, even in the context of the growing storm is not know because the people with knowledge of such data perished in the storm (Lakescan, 2017). The risks and disaster were not just contained to the Cawarra, people in land also were injured, lost their property or perished because of the disaster. Some issues noticed are the lack of storm preparation, weather prediction facilities, identification of hot spots that could helped identify high cluster populations, ways to identify high risk population like the elderly, children etc (Craton and Saunders, 2000).

The SS Cawarra tried to enter the port and this was one more major reason for the disaster. While, the reasons on why the captain tried to enter the port amidst the gale is not documented, there should have been some storm protocol for the captain to follow when caught in the gale. An alternative protocol where the ship could have attempted docking at a calmer port should have been drawn. The problem of Cawarra’s failures was analogous to a Norwegian freighter Sygna. Sygan was one among many ships waiting to enter the port and did not heed or was not able to listen to weather prediction warnings (Duncan, 2014). During the course of the storm, as the Cawarra was tossed in the sea waves, it was expected that the ship would launch its life boats, but the life boats were delayed. Only one of the persons on the ship survived and he later stated that there was no panic even as the crew left the deck to go inside, when the waves washed the deck. A lack of urgency could hence be one of the reasons for the failure of launching lifeboats in time. This consequentially led to another failure, where the lifeboats when they were launched at last were too late.
